Installation Request

Please complete the form provided below so that we can best assess your needs. All items marked with * are required.

All questions must be answered in order to complete the form.

Tank has been backfilled and is installed above finished grade? (12" EXT. MAX)

Tank is completely empty and free from all debris? (NO LIQUID)

Center of inlet pipe is 30" (min) from the bottom of the tank?

Inlet grommet is installed correctly using a hole saw?

Discharge piping size is 1.25" from the tank to the sewer tap?

1” ELECTRICAL adapter installed on tank? (THREADS IN / SOCKET OUT)

1” ELECTRICAL conduit (SCH 40 PVC) installed from the tank to the panel? (ELECTRICAL FITTINGS ONLY)

A pull string is provided through the conduit from the tank to the panel?

The control panel is mounted 40" to 60" above finished grade?

All conduit penetrations are on the bottom of the control panel?

Dedicated 30 AMP circuit feeding the control panel? (LABELED)

Verify four #10 gauge wires are connected. (2 hots, 1 ground, 1 nuetral)

Verified that the voltage at the control panel is between 216V and 264V?

All residential / main sewer valves are accessible and in the open position?

Water is available for testing the pump? (WE DO NOT OPEN WATER MAIN VALVES)

This form should not be submitted unless the work above is 100% complete.
On the installation date we will not wait for you or your contractor to finish.
If the installation cannot be completed for reasons covered in questions 1-15 the installation will be cancelled. A new request form must be submitted once the corrections have been made.
A return trip fee of $303.75 will be backcharged to the builder.
