Project Start-Up

Please complete the form provided below so that we can best assess your needs. A representative from Hydro Source will then contact you to schedule the requested service. All items marked with * are required.

Please confirm the following information by selecting the coordinating fields.

Tank has been backfilled and is installed above finished grade? (12" EXT. MAX)

Tank is completely empty and free from all debris? (NO LIQUID)

Center of inlet pipe is 30" (min) from the bottom of the tank?

Inlet grommet is installed correctly using a hole saw?

Discharge assembly is installed per factory diagram? (VALVE TO TANK)

Mushroom vent is installed?

1" tank adapter and cord grip installed on the inside of the FRP tank?

1" electrical PVC conduit installed from the tank to the panel?

The control panel is mounted 40" to 60" above finished grade?

All the conduit penetrations are on the bottom of the control panel?

The conduit penetrating the panel is sealed using duct seal?

Dedicated 30 AMP circuit feeding the control panel? (LABELED)

Verify four #10 gauge wires are connected. (2 hots, 1 ground, 1 nuetral)

Verified that the voltage at the control panel is between 216V and 264V?

Confirm that there is no moisture inside the EQD?

The EQD plug must be able to extend past the top of the tank? (24" MAX)

The rope is secured with a ziptie to the top of the tank?

Discharge piping size is 1.25" from the tank to the tap?

All residential / main valves are opened before start-up?

All electrical, operational and start-up test performed as directed in the manufacturer's installation manual?

Start-up Approved?
