This form should not be submitted unless the work above is 100% complete.
On the installation date we will not wait for you or your contractor to finish.
Once Hydro Source Services, Inc has received the request form, the installation will be scheduled for the following Wednesday (Request forms must be received by 12pm Tuesday).
Installations will start at 7am on Wednesday. Please be prepared.
If the installation cannot be completed for reasons covered in questions 1-15 the installation will be cancelled. A new request form must be submitted once the corrections have been made.
A return trip fee of $250.00 will be backcharged to the builder.
